Pain in the shoulder joint

Pain in the shoulder joint

Pain in the shoulder joint

Pain in the shoulder joint and shoulder pain can go beyond work ability and quality of life. Pain in the shoulder joint can be due to less serious causes such as inflammation, muscle damage / myalgia, stretching, joint locking, tendon damage, nerve irritation in the neck - other diagnoses can be frozen shoulder or bursitis - but keep in mind that it can also, in rare cases, be due to more serious problems.

Some of them most common diagnoses is (thankfully) muscular dysfunction (so-called myalgias) in combination with joint restrictions (also known as facet joint locks) in the thoracic spine, the costal joints (the rib joints that attach to the thoracic spine), in the neck and the transition to the neck - in this area are especially trapezius, levator scapulae and pectoralis known to cause pain in the shoulder joint.

Of the relevant shoulder muscles that are often hurt by incorrect use are mentioned subscapularis, infraspinatus, supraspinatus and teres minor. It is therefore important to analyze / evaluate these carefully and find out which ones are too active and which ones are too weak.

More serious, but rare, diagnoses can be lung disease, pneumothorax (collapsed lung), heart problems, metastasis (spread of cancer) or pulmonary embolism. Pain in the shoulder joint can occur on both the left and right side, and towards the front part on the front of the shoulder (against acromion in what we call the AC joint, which stands for the acromioclavicular joint).

Causes can be overload, trauma, fall, accident, wear and tear / Osteoarthrtitis (joint wear), muscular failure loads (especially in the chest, shoulder and neck muscles) and mechanical dysfunction in the adjacent joints (eg neck, chest, ribs and / or shoulder). If there has been a fall from a bicycle or similar with subsequent pain, it should be investigated for fractures or leg injuries with imaging.

- When the rotator cuff muscles give pain in the shoulder joint

A common diagnosis that causes pain in the shoulder joint is rotator cuff injuries or muscle dysfunctions (rotator cuff is the stability muscle of the shoulder). Several of these are known to refer to pain in the shoulder joint. Pain in the shoulder joint can also occur or worsen in combination with the diagnosis rib lock - which occurs when a joint gap in the thoracic spine, a so-called thoracic-costal joint, becomes very restrictive in the movement with associated muscle tension. This can cause very sharp pain, within either the left or right shoulder blade, which almost runs across the back - from back to front - sometimes towards the shoulder joint.

If the pain is more localized to the upper part of the shoulder joint, then one will often see an associated restriction and stiffness in the cervicotoracal joint (also called CTO - where the neck meets the thoracic spine) and the shoulder - this will also be able to give local, high muscle tension in, among other things, supraspinatus and subscapularis musculature.

Who is hurt by the shoulder joint?

It is especially craftsmen and those who expose the shoulder to heavy, repetitive strain that are affected by shoulder joint pain. But pain in the shoulder joint also affects those who are too inactive and have too little support muscles - then a smaller load such as. raking in the garden causes the shoulder and neck to lock completely with associated muscle tension.

- Can affect both old and young

Shoulder pain is a condition that affects a larger proportion of the population at some point in their lives - both old and young. It is important that you take the pain seriously and do something about it - otherwise it can be repeated and worsen. If, in addition to pain in the shoulder joint, you have chest pain and a family history of heart problems, it is a good idea to get an examination from your GP to check that everything is fine, just to be on the safe side. Fortunately, the most common cause is poor function in nearby muscles and joints.

What can I do even against shoulder pain?

1. General exercise, specific exercise, stretching and activity are recommended, but stay within the pain limit. Two walks a day of 20-40 minutes make good for the whole body and sore muscles.

2. Trigger point / massage balls we strongly recommend - they come in different sizes so you can hit well even on all parts of the body. There is no better self help than this! We recommend the following (click the image below) - which is a complete set of 5 trigger point / massage balls in different sizes:

trigger point balls

3. Training: Specific training with training tricks of various opponents (such as this complete set of 6 knits of different resistance) can help you train strength and function. Knit training often involves more specific training, which in turn can lead to more effective injury prevention and pain reduction.

4. Pain Relief - Cooling: Biofreeze is a natural product that can relieve pain by cooling the area gently. Cooling is especially recommended when the pain is very severe. When they have calmed down then heat treatment is recommended - it is therefore advisable to have both cooling and heating available.

5. Pain Relief - Heating: Warming up tight muscles can increase blood circulation and reduce pain. We recommend the following reusable hot / cold gasket (click here to read more about it) - which can be used both for cooling (can be frozen) and for heating (can be heated in the microwave).

6. Prevention and healing: Compression noise like that like this can increase blood circulation to the affected area, thereby speeding up the natural healing of injured or worn muscles and tendons.

Where is the shoulder joint?

The shoulder joint is the joint that attaches the upper arm (humerus) to the shoulder blade via the shoulder cavity (glenoid cavity).

Anatomy of the shoulder joint

Anatomy of the shoulder joint

Here we see the important anatomical landmarks around the shoulder joint. We see how it attaches to both the glenoid cavity (shoulder cavity) and the acromion.

Muscles around the shoulder joint

We say that there are 6 muscles that have the main responsibility for stabilizing the shoulder joint. These are the biceps brachii, triceps, supraspinatus, infraspinatus, subscapularis and teres minor. The shoulder does not have significant support from tendons and ligaments, so it is extra dependent on good muscle function to avoid injuries - which in turn emphasizes the importance of keeping the shoulders and thoracic spine at optimal function. Here you see an illustration with muscle attachments.

Muscles around the shoulder joint

There are also a number of joints that attach or are related to the shoulder joint - of the most important are the cervicotoracal junction (CTO), C6-T2 (which includes the two lower cervical vertebrae C6-C7 and the two upper thoracic vertebrae T1-T2). In the absence of function in these, joint pain and associated myalgias can occur in nearby muscle attachments. The joints T1, T2, T3, T4, T5, T6, T7, T8, T9, T10 - ie 10 of the 12 thoracic vertebrae - also have a direct effect on the shoulder blade and shoulder.

Pain in the neck

- The body is complex… and fantastic!

As we note from the pictures above, the body's anatomy is both complex and fantastic. This, in turn, means that we must focus holistically on why the pain occurred, only then can effective treatment be provided. It is also important to remember that it never does 'just muscular', there will always be a joint component, an error in movement pattern and behavior that also forms part of the problem. They only work together as a unit.

What is pain?

Pain is the body's way of saying that you have injured yourself or are about to hurt you. This is an indication that you are doing something wrong. Not listening to the body's pain signals is really asking for trouble, as this is its only way to communicate that something is wrong. This applies to pain and aches all over the body, not just back pain as so many people think. If you do not take the pain signals seriously, it can lead to long-term problems, and you risk the pain becoming chronic. Naturally, there is a difference between a little tenderness and pain - most of us can tell the difference between the two.

Treatment and specific training guidance by a musculoskeletal expert (physical therapist, chiropractor or manual therapist) is often advised to overcome the problem for a long time. The treatment will target and treat dysfunctions in the muscles and joints, which in turn will reduce the incidence of pain. When the pain is attenuated, it is necessary to weed out the cause of the problem - maybe you have a slightly bad posture that leads to some muscles and joints being overloaded? Unfavorable working position? Or maybe you do not perform the exercises in an ergonomically good way?

Pain in the shoulder joint 2

Imaging diagnostic examination of the shoulder joint

Sometimes it may be necessary imaging (X, MR, CT or diagnostic ultrasound) to determine the exact cause of the problem. Normally, you will manage without taking pictures of the shoulder joint - but this is relevant if there is a suspicion of injury, fracture or serious pathology. In certain cases, X-rays are also taken with the intention of checking the curvature of the back, then with a view to examining for scoliosis or Scheuermann's (sharply increased kyphosis). Below you see various pictures of what the shoulder joint looks like in the various forms of examination.

“R: Nothing pathologically proven. No discoveries. "

Explanation: This is a composition of MRI examination images from a normal shoulder without MRI findings. The shoulder was sore, but there were no injuries visible in the pictures - it turned out later that the pain came from joint restrictions in the neck and chest, as well as active muscle knots / myalgias in the rotator cuff muscles, upper trapz, rhomboidus and levator scapula.

The solution was stabilizing rotator cuff training (see exercises), chiropractic joint correction, muscle therapy and specific home exercises. Thank you for sharing such photos with us. The photos are anonymized.

Time classification of pain in shoulder joint. Is your pain classified as acute, subacute or chronic?

Pain in the shoulder joint can be divided into acute (sudden), subacute og chronic (prolonged) pain. Acute shoulder joint pain means that the person has had pain in the shoulder joint for less than three weeks, subacute is the period from three weeks to three months and the pain that has a duration of more than three months is classified as chronic.

Clinically proven effect on relieving pain in the shoulder joint

A systematic meta-analysis published in 2010 (Kalichman) found that dry needling can be effective in treating musculoskeletal pain problems. Chiropractic treatment, consisting of neck mobilization / manipulation and specific home exercises, has a clinically proven effect on relieving neck pain. A recent study published in the renowned journal Annals of Internal Medicine (Bronfort et al, 2012) showed that this form of treatment had a documented better effect compared to medical treatment in the form of NSAIDs Non-steroidal anti-inflammatory drugs. As mentioned, neck function is often directly linked to shoulder and shoulder joint pain.

Conservative treatment of shoulder pain

home Practice is often printed and used to address improper use of musculature, with the intention of providing a long-term, long-lasting effect.

Ultrasound can be used both diagnostically and as ultrasound therapy, the latter works by providing a deep-warming effect aimed at musculoskeletal problems.

Joint Mobilization or corrective chiropractic joint treatment increases the movement of the joints, which in turn allows the muscles that attach to and near the joints to move more freely. Chiropractic joint treatment is often combined with muscle work in the treatment of chest problems.

Stretching can be relieving for tight muscles - Photo Seton
Massage and muscular work It is used to increase blood circulation in the area and thus reduce muscular tension, which in turn can cause less pain.

heat treatment used to give a deep-warming effect on the area in question, which in turn can give a pain-reducing effect - but it is generally said that heat treatment should not be applied to acute injuries, as is ice treatment, e.g. Biofreeze, to be preferred. The latter is used for acute injuries and pains to help ease the pain in the area.

Shockwave Therapy is effective against a variety of tendon injuries that can hit the knee.

laser treatment (also known as anti-inflammatory laser) can be used at different frequencies and thus achieve different treatment effects. It is often used to stimulate regeneration and soft tissue healing, plus it can also be used anti-inflammatory.

List of treatments (both much alternative and more conservative):

Chiropractic treatment for shoulder joint pain

The main goal of all chiropractic care is to reduce pain, promote overall health and improve quality of life by restoring normal functioning of the musculoskeletal system and nervous system. In case of shoulder joint pain, the chiropractor will treat the shoulder locally to reduce pain, reduce irritation and increase blood supply, as well as restore normal movement in the neck, thoracic spine and shoulder blades. When choosing a treatment strategy for the individual patient, the chiropractor places emphasis on seeing the patient in a holistic context through thorough history (clinical history) and clinical examination. If there is a suspicion that the shoulder joint pain is due to another disease, you will be referred for further examination. It may also be relevant Shockwave Therapy, needle treatment or other modalities.

The chiropractor treatment consists of a number of treatment methods where the chiropractor mainly uses his hands to restore normal function of the joints, muscles, connective tissue and nervous system:

- Specific joint treatment
- Stretches
- Muscular techniques
- Neurological techniques
- Stabilizing exercise
- Exercises, advice and guidance

What does one do chiropractor?

Muscle, joint and nerve pain: These are things that a chiropractor can help prevent and treat. Chiropractic treatment is mainly about restoring movement and joint function that can be impaired by mechanical pain. This is done by so-called joint correction or manipulation techniques, as well as joint mobilization, stretching techniques, and muscular work (such as trigger point therapy and deep soft tissue work) on the involved muscles. With increased function and less pain, it may be easier for individuals to engage in physical activity, which in turn will have a positive effect on both energy, quality of life and health.

Surgery of the shoulder joint

Surgery and pain injections are some of the invasive treatments you want to stay away from, but in some cases they are necessary. For example, in the case of a collarbone fracture, it is necessary to operate on the bone in place (if it is a complicated fracture), so that it heals properly. In this example, a cyclist was unlucky and broke his collarbone - he had to have surgery. Here you see before and after the picture. The orthopedic surgeons had to operate a 7-screw titanium plate to make sure the fracture healed properly. Can you imagine what that collar bone would look like if it hadn't been operated on? It hadn't looked pretty.

Arthroscopy of the shoulder joint

Recent research and studies have shown that you should not undergo arthroscopy (scraping) of either shoulder or knee - if you have any choice. Yes, it provides short-term relief in many cases - but in the long run it can cause the pain to be worse than it was in the beginning. This is due, among other things, to scar tissue and damage tissue after such an operation. They now strongly recommend exercise and treatment in combination as an alternative.

Exercises and training for shoulder pain

An expert in muscle and skeletal disorders can, based on your diagnosis, inform you of what ergonomic considerations you need to take to prevent further damage, thus ensuring the fastest possible healing time. After the acute part of the pain is over, in most cases you will also be assigned home exercises that also help to reduce the chance of relapse. In chronic conditions it is necessary to go through the motor movements you do in everyday life, in order to weed out the cause of your pain occurring time and time again.

Kjerringråd against pain in the shoulder joint

We choose to bring some advice against the pain in the shoulder joint. We have also tried to understand the meaning behind them and thus put a little explanation in brackets. Do not take them too seriously, but remember that they have survived the tooth of time for a reason.

- Drink ginger tea (Ginger reduces muscle pain)
Rest in the sun (The sun provides the basis for vitamin D. Vitamin D deficiency has been linked to increased muscle pain)
- red bell pepper powder (Red bell pepper has among the very highest content of vitamin C - needed for soft tissue repair)
- Eat blueberries (Blueberries have a pain-relieving and anti-inflammatory effect)
- Eat onions and garlic (This one we're not sure about, but maybe again it's the focus on anti-inflammatory action?)

Frequently asked questions about shoulder pain:

Has been diagnosed with osteoarthritis of the shoulder joint / shoulder joint. What should I do?

Osteoarthritis and wear and tear changes do not change much. It affects all of us over time - some more than others. The general recommendations will still be to exercise according to ability and medical history, exercise and keep moving, eat right and try to maintain a healthy lifestyle. A lifestyle change that can make a big difference for those with shoulder pain is to quit and smoke - because it is the case that smoking limits blood circulation to areas that already have less circulation. This in turn leads to less repair and healing in the affected areas. If you have a lot of pain with the shoulders, it can also be useful to see clinics for evaluation of the condition - who may also recommend pressure wave treatment (for calcification in the shoulder joint or tendons) and joint mobilization / manipulation.

Q: Cause of sudden pain in the shoulder joint?

As mentioned, there are a number of possible causes and diagnoses of pain in the shoulder joint on the left or right side - the symptoms must be seen in full. However, among other things, referred pain from nearby muscle dysfunction or joint restrictions (in the neck, thoracic spine, ribs and shoulder) can cause pain in the shoulder joint. Frozen shoulder and subacromial bursitis are also two relatively common diagnoses, but they often come over time. Other more serious causes are lung disease and many other diagnoses. See list higher up in the article. If you elaborate on your concerns in the comments section below, we can do more to help you.

Q: Can foam rolls help me with shoulder pain?

Yes, a foam roller can help you with stiffness and myalgias, but if you have a problem with the shoulder joint, we recommend that you contact a qualified health professional in the field of musculoskeletal subjects and get a qualified treatment plan with associated specific exercises - most likely so will You also need joint treatment to normalize the condition. Foam roller is often used against the thoracic spine to increase circulation in the area. We otherwise recommend that you go for daily walks with good arm swings to keep your blood circulation up - there are no shortcuts to good health.

Also, remember that cold treatment can provide pain relief against sore joints and muscles. Blue. Biofreeze (you can order it here), which consists mainly of natural products, is a popular product! Contact us today via our Facebook page, then we'll fix one discount coupon for you!

Q: Why do you get shoulder pain?
Pain is the body's way of saying that something is wrong. Thus, pain signals must be interpreted as meaning that there is a form of dysfunction in the area involved, which should be investigated and further remedied with proper treatment and exercise. Causes of pain in the shoulder joint can be due to sudden incorrect loading or gradual incorrect loading over time, which can lead to increased muscular tension, joint stiffness, nerve irritation and, if things have gone far enough, damage to joints and tendons.

Q: Man, 52, asks - what should be done with a sore shoulder joint full of muscle knots?

muscle knots have most likely occurred due to a misalignment of the muscles or a misalignment. There may also be associated muscle tension around joints in the nearby chest, ribs, neck and shoulder joints. Initially, you should get qualified treatment, and then get specific exercises and stretching so that it does not become a recurring problem later in life. You can also use the following exercises to Exercise chest and shoulder stability. Contact us for more tips and exercises.
