Cuboid syndrome / subluxation - Image, treatment and cause

 

Cuboid syndrome, also known as cuboid subluxation or cuboid locking, is often caused by a single trauma, such as overtraining or wiggling the foot - or with repetitive strain without enough healing. Athletes are most often affected.

It then usually occurs due to loads that put pressure on the calcaneocuboid joint (where the heel meets the cuboid), which we find on the outside of the foot.


One theory is that the joint and ligaments become overloaded and that a lock in the cuboid joint occurs, with associated problems in the main ligament and the supporting muscle (especially the peroneus may be exposed).

 

Treatment of cuboid syndrome involves relief from the causative agent, rest, mobilization / manipulation (a specific joint adjustment to put the joint in place - this should only be performed by a manual therapist or chiropractor) and training of the muscles involved, as well as possible sole adjustment by an orthopedist to straighten the arch of the foot - exercises to strengthen the arch of the foot can also help prevent the recurrence of the problem.

What are the patient's symptoms?

Pain on the outside of the foot. Often the patient will report that they have pain at a specific point in the foot, they will then like to point directly at the cuboideus, and report that this feels locked - the words 'locking in the foot' are often used.

 

Treatment modalities: Evidence / studies.

A meta-study published in Sports Health (2011) concluded that manipulation / joint correction should be the first step in the treatment of cuboid syndrome / cuboid subluxation. This is to restore normal movement in the joint, which in turn provides more correct use of the foot and ankle.
