Primary sclerosing cholangitis

Primary sclerosing cholangitis is similar Primary biliary cirrhosis an autoimmune condition that leads to inflammation and inflammation of the liver's transport channels. This can have fatal consequences as toxins and bile accumulate - and can lead to scarring, liver failure and liver cancer.

Symptoms of primary sclerosing cholangitis

The disease causes symptoms associated with liver problems, such as fatigue, itching, enlarged liver and jaundice (among other things, the sclera in the eyes turns yellow).

If the condition is not treated, the symptoms will gradually worsen - and can lead to complete liver failure. In later stages, one can see fluid accumulation in the stomach, enlarged liver and other serious symptoms that the liver is about to fail.

Clinical signs

As mentioned above under 'symptoms'.

Diagnosis and cause

The diagnosis is made through a series of clinical examinations (including blood tests), lab tests, urine analysis and thorough history. Particular attention is paid to certain antibodies in the blood test. Special imaging is used for imaging MRI examination to visualize the liver called MRCP.

The cause of primary sclerosing cholangitis is unknown (idiopathic), but it is known that there is an autoimmune response in the disease picture.

Who is affected by the disease?

Like most other liver diseases, women are affected more often than men (3: 1). Approximately 0.22 - 8.5 per 100000 are affected. You also see that they with Ulcerative colitis and other abdominal diagnoses are more often affected. The disease can affect men and women of all age groups, but the most commonly afflicted occurs at the age of 40. The disease is often asymptomatic long before it results in clinical manifestations. Among other things, the well-known snowboard driver Chris Klug had to have a liver transplant due to this clinical condition.

Treatment

Drug treatment can slow the symptoms, but there is no cure for this disease. Liver transplantation may be necessary if the condition has led to severe liver dysfunction or liver failure.

The most common form of treatment for autoimmune conditions is included immunosuppression - that is, drugs and measures that limit and cushion the body's own defense system. Gene therapy that limits inflammatory processes in immune cells has shown great progress in recent times, often in combination with increased activation of anti-inflammatory genes and processes.
